I need help to implement an important online Caring Network Respite Registry serving children and adults with disabilities and the elderly population Statewide? This will be an online database accessible to registered
(paid) users from my existing website.

I need several components:

Registry front page from main website with our "How To Use" Info and Disclaimer Info etc. This page contains 2 hot buttons (we design), one for the parent to access the provider database and one for the respite provider to enter data that can be searchable. We need a back-end to make provider record active or inactive and to manually update or change info in fields.
Maybe you can think of this as an employee/employer database

1) The Respite Care Provider Registry/database with displayed info about the provider-see below

2) Parent/Family joins database as a member, pays a small membership fee to access it-see below

Respite Care Provider Registry: Contains an editable form is filled out by the Respite Care provider about themselves ...with submit button• An automatic confirmation by an e-mail to the person filling it out with hot link to a pdf form posted on our site (welcome to the Respite Registry) • Listing info display for viewing• They can get into their own listing to update/change information with a user code and pass word (this is optional, for now we could edit manually from behind the scenes)

Parent/Family or recipient• Sign up as member with user code and password, pay through Caring Community gateway• An automatic confirmation by an e-mail to the person filling it out with hot link to a pdf form posted on our site (Welcome to the Respite Registry) • Can access this database anytime they want with user code and pass word• Database of provider records is (ideally) searchable by zipcode/ miles from searcher's home• Display of information to be in a form, one form per registered provider. Form info could be displayed 20 or 30 to a page, or we could have more abbreviated listings on a page with the complete record info listed in a rollover or pop-up window• Search on other criteria? To be determined

To recap, we need a database of providers, who could ideally register for free on line (NOTE: we would still need to approve a listing manually), names and info would be posted on line in a listing format. The info is searchable by at least zip code, ideally distance from searcher's location or zip code, accessible ONLY by paid members (one time fee, no renewals).
